Camera won't focus

Camera won't focus on anything beyond 4 inches from the phone anything out from that is blurry. any ideas what that is?

I've seen this and the lens was scratched or dirty

Try installing Google camera off Play Store and try that... Should hopefully point to either hardware or software..
